Flu Vaccination Form 

Please only use this form if you do not have access to the NHS app, Patient Access app or myGP app.

If you have any of those apps, it's much easier and quicker to find a flu jab appointment on the app. If not, fill the form to get reception to call you back to book the flu jab.

Who is eligible?

The flu vaccine is given free on the NHS to people who:

  • are 50 and over, including those who'll turn 50 in the 6 months following the start of the flu season
  • have certain health conditions
  • are pregnant
  • are in long-stay residential care
  • receive a carer's allowance, or are the main carer for an older or disabled person who may be at risk if you get sick
  • live with someone who is more likely to get infections (such as someone who has HIV, has had a transplant or is having certain treatments for cancer, lupus or rheumatoid arthritis)
  • frontline health or social care workers
  • Children are also eligible, most will be offered the vaccination in school
